Healthcare delivery in the United States is at a crossroads. Without improving access, better coordinating care, aligning incentives, and integrating information systems, healthcare expenditures will grow while outcomes will remain highly variable. Vehicles such as accountable care organizations, medical homes, retail clinics, and bundled payments all seek to motivate us to achieve the value imperative in today’s fragmented healthcare delivery system.

So, who is doing it well and how are they doing it?

This conference will address those questions by introducing us to organizations and individuals who have proven that catalyzing innovation and engaging patients and their families can allow us to radically redesign our healthcare delivery system.
